Medicare rebates can be accessed under a Medicare Chronic Disease Management Plan (formerly Enhanced Primary Care Plan). Chronic Disease Management plans are a government incentive which allows individuals with a chronic medical illness or condition to receive Medicare rebates for allied health services, such as exercise physiology.
You are eligible for a Medicare Chronic Disease Management Plan if you have a health condition of prolonged nature (3 months or longer). To determine if you are eligible for a Chronic Disease Management Plan consult your doctor.
